When will I know if surgery is absolutely necessary?

I use Sodium Chloride drops twice a day. I also have mild cataracts, a split in the right optic, the gel behind the right eye ruptured which causes a film over the eye. When will I know surgery is absolutely necessary. I'm very paranoid about having any type of eye surgery.

Are you considering cataract surgery? And do you also have macular degeneration?
